If you are not an existing patient at the Eyecare Plus practice you select on the order form, you will need to fax your prescription or advise the name & phone number of your optometrist for prescription verification. prescription information

Prescription must be dated within the last 6 months to order extended wear contact lenses (Focus Night & Day).

* Coopervision Clear Choice lenses can only be supplied to patients who are ordering from the practice who prescribed them.

 

GST NOTE

  • All prescription contact lenses are GST free.

  • Coloured contact lenses with NO optical power [Plano] attract GST of 10%, in addition to the listed price.
